Event Registration form specific to the event
Note: The Trace has a Registration and Roster Form specific to the Trace in the packet. The Pow Wow has a Registration and Roster Form specific to the Pow Wow. The JLDA does not have a Outpost Registration Form. Registration is done online by each person separately.
Leaders
Leaders need to contact Steve Monez annually to be checked against the Staff Exclusion List (SEL) and the National Sex Offender Public List (NSOPL)
Visiting Adult

Boys
New York State Public Health Law requires that a parent or guardian of campers who attend an overnight children’s camp for seven (7) or more consecutive nights, complete and return the following form to the camp representative.
